Copy of certified birth certificate
Copy of social security card
Current photo of student  (Kindergarten-Level 1)

  1. (no larger than wallet-size)

Current physical examination

  1. (Florida Department of Health Form DH3040)

Immunization records

  1. (Florida Department of Health Form DH680)
  2. Kindergarten and 7th grade students (must have specialized shots)

Most recent standardized test results
Previous report card
IEP or 504 (if applicable)
Completed transcript

Please submit the following documents to the Academy office.

  1. Copy of current IEP or 504 Accommodation Plan
  2. Copy of students recent report card
  3. Copy of students standardized testing report

**An initial review of student plan is required before the application process begins. We need to determine academic eligibility prior to family interview.

Take special note to our school's policies and procedures. After the administration has considered the application, an interview will then be scheduled with the school's principal and office administrator for both the student and parents.

After the interview process, the administration office will notify you by phone whether or not your application is accepted or denied. It is in the best interest of the school and the families involved that F.S.C.A. screens all families during the enrollment process to create the best learning atmosphere for its students and staff.
